We are part of The Bury Local Care Organisation (Bury LCO) formally launched on 1 April 2019, although we have been developing over the last year or so. The aim of the LCO is to enable improvements in population health and wellbeing, by providing improved health and care services to the people of Bury. We are an innovative formal alliance of organisations: Bury Council; Bury GP Federation; BARDOC; Bury Voluntary, Community and Faith Alliance; Northern Care Alliance NHS Group (Salford Royal and Pennine Acute Hospital NHS Trusts); Pennine Care NHS Foundation Trust; Persona Care and Support Ltd. The LCO partners have made a commitment to work together as a single new body within the public sector, responsible for the transformation and delivery of a group of services and improving people’s health and wellbeing The Bury LCO will work as one joined-up team rather than separate organisations, leading to more coordinated help and support, getting the best value for public money. We are looking to recruit a social care officer and have a vacancy in our Integrated Health and Social Care Team based at Tottington Medical Centre This is an exciting opportunity to join as a social care officer and be involved in this innovative approach to working differently and making a difference for the people of Bury. As a Social care officer you will make a positive contribution to the work and development of this integrated journey. The Integrated Neighbourhood Teams are community based working as multi-disciplinary teams that can make a real difference to the lives of adults living within the Bury area. You will encourage and motivate customers and assist them to build their confidence and achieve their personal goals. You will need to demonstrate enthusiasm, commitment, drive and a strong teamwork ethic and be flexible in supporting colleagues and the service during busy times and periods of absence. You will be supportive in assisting and promoting the community services. You will be able to build rapport and trust to establish effective relationships with people to support, engage and enable them to identify their strengths. This will be achieved through meaningful conversations and assessment. You will use an asset and strengths based approach, enabling people to take control of their own lives and focusing on what is important to them in to manage their own health and social care needs better If you are highly enthusiastic, energetic and a great team player we would especially like to hear from you. Working hours for this post are 37 per week, Monday to Friday (core hours 8.45am – 5pm).
